When it comes to home care, it is important to know when you or your loved one should start looking for the proper home care services. He or she may be recovering from an unfortunate accident and are in need of a nurse or personal assistant while recovering at home. Some of us in the aging population may be struggling with simple tasks, such as daily errands and activities, and are in need of some assistance. Some of us may be in the best physical condition but would still appreciate the help from a companion or a personal aide.

There are countless reasons why someone might require home care, and knowing exactly what he or she needs can sometimes be confusing and difficult to understand. Therefore, we at Community Home Health Care, are here to help lay your concerns to rest. We have been around for over 30 years and offer many services to relieve discomfort and stress at home. From a full-time live-in to part-time assistance, we are here to help provide the proper care necessary to improve your quality of life.

“Services are funded in part by the Administration on Aging, and New York county offices for the Aging Contributions are voluntary and confidential and are used to expand the services for which they are received. No one will be denied services because of an inability or unwillingness to contribute. Suggested contributions level should equal the cost of the service for participants who self-disclose income at or above 185% of Federal Poverty Level.”

Therapy Program

Physical therapy, occupational therapy, and speech / language pathology services provided through the agency will:

  • Analyze and interpret patient’s needs on the basis of the medical history, pertinent precautions, limitations and evaluative findings
  • Identify short and long-term goals that are measurable, objective and related to functional mobility as determined by medical history and evaluative findings
  • Formulate a treatment plan to achieve the goals identified
  • Review physician’s request for treatment, and if appropriate, recommend revisions on the basis of evaluative findings

Nurses: RN's

  • Diabetic care and nutrition counseling
  • Wound care
  • Medication management and teaching including injections
  • Colostomy care
  • Assesses patient health status, makes recommendation and referrals for PT/QT/SPT/Social workers and other medical providers
  • Assist physician in the evaluation of a patient’s level of function
