Many private insurance companies offer home health care services as a benefit. We will check with your carrier to determine benefits provided.

Medicare is a federal health insurance program for people age 65 or older and certain disabled people under 65. It provides care and support services to individuals who have home health care needs for a limited duration. CHC has contracts with several Certified Home Care Agencies (CHHAs) who service the clients with home health aides.

Medicaid is the medical assistance program for low-income people of all ages who are unable to pay for care and who meet the eligibility guidelines. It is financed by federal, state and local funds, and the services covered are set by federal and state law. CHC has contracts with Rockland County and Orange County Departments of Social Services for services through the Medicaid Personal Care and Private Duty Nursing Programs.

Also known as Nursing Home Without Walls, the Long Term Home Health Care program operates under a specific federal Medicaid waiver. It may be part of a CHHA, or be nursing-home-based. The program offers health care and support services to the disabled and chronically ill who are medically eligible for admission to a nursing home, but who choose to be maintained at home. CHC has contracts with several LTHHCPs that service the clients with personal care aides.

MLTC provides health and long-term care services to adults with chronic illness or disabilities, to better address their needs and to prevent or delay nursing home placement. Services include – but are not limited to – nursing, physical therapy, occupational therapy, speech pathology, medical equipment and supplies, podiatry, dentistry, optometry, respiration therapy, transportation, and social day care. This program is available to Medicaid recipients who are eligible for admission to nursing homes. CHC has contracts with several MLTCs that offer personal care aides.

Services are funded in part by the Administration on Aging, and New York county offices for the Aging Contributions are voluntary and confidential and are used to expand the services for which they are received. No one will be denied services because of an inability or unwillingness to contribute. Suggested contributions level should equal the cost of the service for participants who self-disclose income at or above 185% of Federal Poverty Level.
